The Willows: A Short- and Long- Term Skilled Nursing & Rehabilitation Community
The Willows provides a full range of high-quality skilled nursing care in a modern, homelike atmosphere. A team of registered nurses, licensed practical nurses, certified nursing assistants, and social workers will partner with you, your loved ones, and your doctor to provide a personalized care plan.
Our rehab therapy team works with your primary care physician to develop and implement a care program to enhance mobility and speed your return home with the best functional outcome possible.
The place to go for short-term rehab or intermediate and long-term skilled nursing care
Long Term
The Willows is a CARF-accredited skilled nursing community licensed to provide medical care. At The Willows, we empower residents and their families to make the essential care decisions that affect their health and well-being and provide as much choice and flexibility as possible in their daily lives.
Residents live in private rooms with shared or private bathrooms. Each floor, which is referred to as a “neighborhood,” features an open country kitchen and common areas that encourage socialization and interaction among age and mobility peer groups.
For residents who need dementia care, we have Woodside Gardens, a memory support neighborhood on a dedicated floor. Woodside Gardens features a multi-sensory room that promotes relaxation or stimulation for residents who may otherwise be unable to participate in traditional therapy or recreation programs.
Our nursing care includes:
- Skilled and intermediate nursing care
- Rehabilitation
- Cardiac and stroke recovery
- Hospice and palliative care

Short Term
After illness or surgery, you want to get back on your feet as rapidly as possible. The Willows rehabilitation team’s individualized therapy plans —physical, occupational, and speech—are designed to get you well, home, and back to your daily life quickly. Our short-term rehab community encourages mobility and fosters independence.
Therapy services are available seven days a week as well as on an outpatient basis after discharge.
Our licensed therapists provide individualized care plans for:
- Joint replacements
- Post fracture and other musculoskeletal conditions
- Stroke recovery and other neurological conditions
- Parkinson’s disease
- General strengthening after hospitalization
- Conditions requiring physical therapy, occupational therapy, or speech therapy

Care Philosophy
The Willows therapists and nurses work closely with you and your physician to create and implement individualized care plans for joint replacements; post-fracture; post-spinal surgery and other musculoskeletal conditions; stroke recovery and other neurological conditions; Parkinson’s disease; and general strengthening after hospitalization.
We use therapy modalities such as electric stimulation, ultrasound, diathermy, and hot/cold packs for strengthening, pain control, wound care, range of motion, and relaxation.

Our dedicated short-term rehab floor has private rooms with full kitchens, bathrooms, and bedrooms designed to feel like home, not a hospital. We offer bright, spacious accommodations and the choice of having delicious meals served in the privacy of your own room or in the lovely dining room nearby.
